Edit/Delete an Evaluation Form

To edit an Evaluation Form:

Note: Once a form is PUBLISHED, you are no longer able to edit the questions. You can only edit when the form is in the DRAFT stage.

1. Go to "Form"
2. Click on the form (draft) you would like to edited
3. Click the "Edit Form" button on the top left corner of the page
4. Click "Save changes"

To delete an Evaluation Form:

1. Go to "Form"
2. Click on the form (draft) you would like to edited
3. Click the "Edit Form" button on the top left corner of the page
4. Click "Archive Form"
